What are the consequences of health policies on the economic and social situation of individuals? Based on a literature review on French and foreign programmes and policies, we set forth a conceptual framework for the analysis of the effects on poverty of several health policies embedded in the National Strategy for the Prevention and Reduction of Poverty (Stratégie Nationale de Prévention et de Lutte Contre la Pauvreté) and the healthcare stakeholders’ national consultation (Ségur de la Santé). Within this framework, healthcare influences poverty via two causality channels. On the one hand, the improvement of the financial accessibility of healthcare reduces healthcare costs that are likely to lead to individuals fall into or remaining in poverty. On the other hand, the improvement in access to healthcare, and thus individuals’ health status, has an impact on individual’s educational level, access to employment, and income, over the life cycle.

Does a Health Shock Modify Tobacco Consumption?

Smoking caused almost 75,000 deaths in France in 2015, killing half of all regular smokers. It is also responsible for 90% of lung cancer cases, and increases the risk of developing a serious form of cardiovascular disease and of contracting chronic obstructive pneumopathy (Bonaldi et al., 2019). To lower the incidence of these diseases, various anti-smoking campaigns have contributed to reducing average tobacco consumption over recent years. However, some parts of the population, in particular unemployed persons and low-income individuals, are still reluctant to change their smoking habits significantly and over the long term (Beck et al., 2015). (…) In this context, it is important to better understand the factors that foster reducing tobacco consumption. (…) Using the longitudinal data from the Gazel cohort, our results indicate that smokers having such health shock reduce their tobacco consumption more than other smokers. Furthermore, this reduction is maintained up to five years after the health shock. We also show that heavy smokers reduce their tobacco consumption more than occasional smokers.

There is debate about the impact of hospital competition on quality of care. On the one hand, economic theory suggests that when prices are regulated, the quality of care increases in competitive markets. On the other hand, economies of scale and the existence of a positive relationship between volume of activity and quality of care argue in favor of concentration of hospital care supply. Using individual data from the Programme de medicalisation des systèmes d'information (PMSI, French national hospital database) over two years (2005 and 2012), we monitor the evolution of competition between hospitals performing breast cancer surgery in France. We use the practice of two innovative surgical techniques as a measure of quality of care: breast reconstruction.

The United States: Innovating Hospital Services at the University of North Carolina Medical Center

Under pressure of increasing demand for healthcare from an ageing population with multiple chronic conditions, France, as other countries, seeks to advance care coordination across primary, hospital and long-term care sectors. Specialists play an essential role in treating patients with chronic conditions, but little attention is given to their organization out of hospitals, and their role in enhancing care coordination and patient-centered care provision.
In order to investigate different ways in which specialists are working out of hospital to integrate primary and social care, we carried out case studies in five countries (England, Germany, Italy, the Netherlands, and the United States). (…) The two case studies in this report describe the innovative services provided by the University of North Carolina Medical Center in the US southern Atlantic state of North Carolina. The first example concerns the investment in teamwork in genital and urology clinic, for improving care for cancer patients, while the second focuses on an initiative that aimed to improve care coordination of patients with chronic renal insufficiency by integrating treatments provided in the hospital and private dialysis centres.

In order to investigate different ways in which specialists are working out of hospital to integrate primary and social care, we carried out case studies in five countries (England, Germany, Italy, the Netherlands, and the United States). (…) This case study on the organization of specialized medicine in Germany presents three MVZs (Medizinische Versorgungszentren), interdisciplinary ambulatory healthcare centers in Berlin area.

In order to investigate different ways in which specialists are working out of hospital to integrate primary and social care, we carried out case studies in five countries (England, Germany, Italy, the Netherlands, and the United States). (…) This case study looks at the organization and functioning of two pioneer integrated care teams models in England where hospital specialists work closely with primary and long-term care providers and provide services outside hospital.

English-French Glossary of Health Economics Terms

This new online edition of the English-French Glossary of terms, concepts and acronyms in health economics has been reviewed and expanded by the IRDES Publications and Documentation teams. In addition to terms from specialized Anglo-Saxon journals, this edition is based on the English translations of IRDES thematic syntheses, Issues in Health Economics, and on the keywords of the MeSH thesaurus (Medline). It also includes a thematic section that classifies the translated terms according to the disciplines involved (economics, sociology, geography, etc.). The translation of French Institutions on Public Health into English is now integrated into the part: Sigles - Institutions.

The Glossary is updated regularly.
