Rencontres
de géographie de la santé
La deuxième édition des
Rencontres de géographie de la santé, dont l'Irdes est
l'un des partenaires, aura lieu à Rennes les 11 et 12
juin 2020. Ces rencontres sont ouvertes à l'ensemble des
personnes intéressées par les liens entre santé et
territoire quel que soit leur profil (« jeunes »
chercheurs comme confirmés, étudiants en master,
doctorants, post-doctorants, acteurs institutionnels,
associatifs, élus, membres de la société civile, etc.)
et leur discipline (géographie, aménagement,
anthropologie, sociologie…).

to State Medical Aid by Undocumented Immigrants in

France decided many years ago to
grant access to healthcare services for undocumented
immigrants when it introduced a scheme known as Aide
Médicale Gratuite (Free Medical Care) and, since 2000,
the State Medical Aid (AME) scheme. The existence of
this kind of scheme does not guarantee, on its own, that
all of the eligible persons neither access the scheme,
nor make use of it. In this paper we analysed access to
State Medical Aid and its determinants based on data
from the 'Premiers Pas' survey, which was carried out in
2019 on a sample of undocumented immigrants.
Only 51% of those eligible actually take up State
Medical Aid. Almost half of undocumented immigrants who
state that they suffer from pathologies requiring
treatment, such as diabetes or infectious diseases, have
no health insurance, neither State Medical Aid, nor any
other health insurance. Access to State Medical Aid is
slightly higher amongst the 10% who declared that one of
their reasons for migration was health. There, is
however, relatively little correlation between coverage
by State Medical Aid and health conditions, aside from
muscular-skeletal troubles. Access to State Medical Aid
primarily increases with length of stay in France. These
results suggest that most of the migrants are not
familiar with the State Medical Aid scheme and were not
all able to fully grasp a complex scheme. Even among
those living in France for five years or more, 35% are
not covered by State Medical Aid.
This third Questions d'économie de la Santé
("Issues in Health Economics"), which focuses on
undocumented immigrants access to State Medical Aid, is
part of a series of three articles. The first article
addresses the history of the health coverage of
undocumented immigrants in France and provides an
overview of the evidence on the State Medical Aid
scheme. The second article describes the methodology and
the sample of the 'Premiers Pas' survey.

Very little is known about State
Medical Aid (Aide Médicale de l'état, AME), the health
insurance programme for undocumented immigrants in
France. What are the social, economic, and health
characteristics of undocumented immigrants covered by
State Medical Aid? Who are those who do not take up and
for what reasons? Does State Medical Aid (AME) provide
insured individuals with effective access to healthcare
services? Recent debate about a possible reform of State
Medical Aid was only based on scant and sometimes biased
information.
The ‘Premier Pas' survey aimed to shed light on these
questions. It was conducted in 2019 with 1,223
respondents in 63 sites and organisations, in inner
Paris and the Bordeaux conurbation. In this paper we
provide a description of the survey sampling and
protocol, the data collection, and a description of the
sample's structure.
This second issue of Questions d'économie de la
Santé (‘Issues in Health Economics') on
undocumented immigrants' access to State Medical Aid,
based on the 'Premiers Pas' survey, is part of a series.
The first outlines the history of this population's
rights in France and presented an overview of the
information and lack of information about the State
Medical Aid scheme. The third is devoted to an analysis
of the take-up of State Medical Aid.

Since its implementation in 2000,
State Medical Aid (Aide Médicale de l'état, AME), a
public health insurance programme for undocumented
immigrants, has provoked intense debate. Some underscore
the need to protect a vulnerable population and the
universality of the right to healthcare coverage in
France, while others perceive the scheme as an abuse of
the social protection system that encourages illegal
immigration. In a context of severe financial
constraints on the healthcare system, issues relating to
the legitimacy, cost, and effectiveness of State Medical
Aid (AME) are becoming pressing. However, information
about illegal immigrants access and use of the scheme
has for a long time been incomplete. The ‘Premier Pas'
project, conducted by the University of Bordeaux and the
Institute for Research and Information in Health
Economics (Institut de Recherche et Documentation en
économie de la Santé, IRDES), aimed to study
undocumented immigrants' access to State Medical Aid and
healthcare utilisation in France.
Three Questions d'économie de la Santé (‘Issues
in Health Economics') present: a description of the
context and issue of the coverage of undocumented
immigrants in France; a description of the ‘Premier Pas'
survey conducted with people eligible for State Medical
Aid; and, lastly, the first findings of the survey on
their access to State Medical Aid. This first article
looks back at the history of undocumented immigrants'
rights to healthcare in France and provides an overview
of the information — and need for information — about
the scheme.
